A wonderful experience. Entire staff was very attentive, considerate to my health issue, very proficient and ready to answer any questions. From the moment you step in the office, you feel that your needs, concerns and medical matters are a priority to every staff member. My sincere thanks to Courtnell that makes the visit enjoyable as soon you step in, Ida and Uriahes for their expertise and medical professionalism. I felt that both Ida and Uriahes medical expertise was 100% directed toward helping me with my medical condition. They really made me feel like they did care deeply for my health...this helped greatly during medical procedure. Patricia very attentive to answering my questions prior and during my visit. Also, the office manager (forgot her name) was very supportive. If I could give them more than five starts I would definitely do so. Once again, amazing personnel that deeply care for your health.

The staff here is phenomenal. They’re very warm, attentive , knowledgeable, and professional. Patricia M is amazing . She’s extremely kind and gentle. She always made sure that I was comfortable, she’s a wealth of knowledge in this particular medical space, and she made me feel like royalty. Urias , who celebrated completion of his bachelor’s degree while I was there ( CONGRATULATIONS AGAIN SIR! ) also did a stellar job. He’s the expert in his craft. Courtnell always greeted me at the elevator with a smile and handshake, and kept the DELICIOUS top shelf bottled water , and organic snacks flowing like milk and honey in heaven. He’s quite the gentleman that always called to check on me , and kept me informed as well via text message. And then the blonde young lady at the desk who handles billing ( forgive me for forgetting your name ) she was greet as well! The office was extremely clean and fresh as were the bathrooms.. Keep up the good work Doctors Studio on Walton in downtown Chicago!!

"This has been my worst experience. I went there for muscle growth and paid $400 for a session, but they did nothing except recommend using BioPro for 3 months. I followed their advice and used it every morning, but saw no results. Despite this, they charged me $1,200. Nothing improved, so I would advise staying away from Forum Health."

Patricia Meredith was fantastic! She was very patient and gentle and make sure you are very comfortable during the entire stay. Cornell it’s great as well! The entire staff at the Chicago location is phenomenal.

I have a very large prostate so it's hard to pee
I went there for their prostate protocol of ozone injections and antibiotics injected directly into the prostate
I paid approximately $7000 total for the prostate program, a detox nutrition products and a type of prostate cancer test.
After four weeks about three treatments I had no results that were beneficial
They told me you have to stick with it and that's gonna cost $23,000 more upfront cash Visa.
I said that's $30,000 I don't Even have any improvement! Can you give me one guy who could say it's Fantastic they cured my prostate issues and I love them. There are none there are none there I could find that say all their prostate problems like a young man,
It's not FDA approved it's not a recognized technique with those zone and antibiotics injected directly and it was discredited in 2018. If this was really the answer they'd be doing this all over the world. But they're not because it doesn't work and it cost a fortune. Even if I was a very very wealthy man I would not do this because it doesn't work there's no Studies no widely used area this is done Thank you Brent Wortell

First Class Experience here. Met with Lauren and the other staff. All were super cool and made everything go smoothly. Really nice people. They know what they are doing. The business is incredibly accommodating and helped me out when other blood donation centers wouldn't. So glad ii found them.

Wonderful experience today at Forum Health! This was my first visit and it was exceptional in every way. Lauren was my phlebotomist and ha a very gentle touch. The official beautiful.

I will be back for sure. The whole staff is friendly and professional.

Second Visit: URAIHES was my phlebotomist on my visit in January. He nailed it and made my whole experience very easy. He has a great personality and He took my mind off of the actual phlebotomy experience, He took the time to apply an alcohol swab around the tape on my arm so it wouldn't hurt when he removed it. I really like him and look forward to seeing him again next month. Thanks Uraihes for making my visit to Forum Health a good one!

The lady who checked me on wa also exceptional and very welcoming. I didn't get her name but she is also amazing.

I spent months of visits, testing, supplements and thousands of dollars here. I tried to make a follow up appointment one day and they told me that I would have to start paying $500 per month membership fee and left me with no ongoing care and no way to continue the prescriptions that their staff had started me on. Unethical medical practice that appears to be primarily financially motivated.
